Structure/Morphology
The pterygomandibular raphe attaches to the buccinators anteriorly and to the superior constrictor muscle of the pharynx posteriorly. It extends from the hamulus of the medial pterygoid plate of the sphenoid to the mylohyoid line of the mandible.
Anatomical Relations
The inferior alveolar nerve lies lateral to the raphe. Therefore, it is the anatomical landmark in the administration of an inferior alveolar nerve block.

Pterygomandibular Space
Thus, a pterygomandibular space abscess can often spread into the submandibular or lateral pharyngeal spaces via the confluence of spaces, or a submandibular or sublingual abscess can similarly spread into the lateral pharyngeal space.
